Case conferencing: Working together to help our neighbors
One way we assist our unhoused neighbors is by hosting regular case conferencing, which serve as a platform for sharing vital information about clients' situations, whereabouts, challenges and assets.
These meetings provide a space for local service providers and case managers to work together as they help clients find the best housing and programs for their needs. Each meeting is tailored for a specific population, ensuring that the most knowledgeable local experts are in the room to provide tailored support and resources for each person.
Through case conferencing, the Anchorage Coalition to End Homelessness encourages collaboration and information-sharing among service providers, ensuring that each client receives effective support. Addressing the unique needs of different groups in our community is part of our coordinated and comprehensive approach to ending homelessness in Anchorage.
Our regular case conferencing meetings are:
Highly Vulnerable Adult Case Conferencing
This case conferencing is designed to overview medically vulnerable clients within the Coordinated Entry system through both a housing and health care lens. Highly vulnerable individuals have these characteristics:
Two or more co-occurring disabilities (AMHTA & HUD Definitions)
ADL/IADL difficulties (Activities of Daily Living, Instrumental Activities of Daily Living)
Unsheltered
55+ (prioritized by oldest to youngest)
Points of prioritization:
Chronic OR oldest touch in HMIS without a 2-year break
Family Case Conferencing
This case conferencing is designed to go over housing referrals for households with minors in their custody (or who plan to have their children move in with them once housed) who currently have referrals to housing programs.
TAY Case Conferencing
The Transitionally Aged Youth (TAY) Case Conferencing is designed to focus on clients ages 18-24 years old who currently have referrals to housing programs.
Adult Case Conferencing
This case conferencing is focused on single adults ages 25+ who currently have open referrals to housing programs.
Veteran Case Conferencing
This case conferencing is designed to overview veterans experiencing homelessness, determine eligibility for HUD-VASH and SSVF services, and review clients’ progress on their paths to permanent housing.
